Add aws_mfa_device = .... to your ~/.aws/credentials.

Then say:

AWS_PROFILE=foo aws-mfa -long-term-suffix=none -short-term-suffix=mfa AWS_PROFILE=foo-mfa aws ec2 describe-instances -region us-west-2
